D형 간염의 원인과 감염 경로
급성 B형 간염이나 만성 B형 간염을 앓는 사람이나, 또는 B형 간염바이러스를 보균한 자가 D형 간염바이러스에 감염될 때 생기는 간염을 D형 간염이라고 한다.
B형 간염을 앓지도 않는 사람이나 B형 간염바이러스를 보균하지도 않은 건강한 사람에게 D형 간염바이러스(Hepatitis D virus)가 감염되어도 D형 간염이 발병하지 않는다.
다시 말하면, D형 간염바이러스, 그 바이러스성 감염만으로 D형 간염이 생기지 않고 반드시 B형 간염바이러스와 함께 감염되어야 D형 간염이 생기게 된다.
D형 간염의 감염 경로
- B형 간염바이러스에 감염되어 있는 사람이 D형 간염바이러스를 보균하고 있는 사람의
- 피나 점액을 접촉하거나,
- B형 간염바이러스가 있는 사람이 D형 간염바이러스에 오염된 피나 피로 만든 약물을 주사로 맞아 치료받거나
- B형 간염바이러스가 있는 사람이 D형 간염바이러스를 보균하고 있는 사람의 점막을 접촉하거나
- B형 간염바이러스가 있는 사람이 D형 간염바이러스를 보균하고 있는 사람이 썼던 불결한 바늘을 살균 처리를 하지 않고 쓰거나
- B형 간염바이러스가 있는 사람이 D형 간염바이러스를 보균하고 있는 사람과 성교를 할 때 감염될 수 있다.
- 잠복기는 2~8주 정도이다.
D형 간염의 증상 징후
- 아무 증상 징후가 없던 급성 B형 간염이나 만성 B형 간염을 앓는 사람이 D형 간염바이러스에 감염되면 B형 간염이 악화되고 D형 간염의 증상징후가 나타날 수 있고 그의 증상 징후가 아주 심하게 급속도로 진행되어 짧은 시간 내 사망할 수 있다. 즉 전격적 D형 간염이 생길 수 있다.
- 황달, 열, 간 비대, 복통, 구토, 기운 탈진, 입맛 상실 등의 증상 징후가 생길 수 있다.
- 때로는 사망할 수도 한다.
D형 간염의 진단
- 증상 징후, 병력, 진찰 등을 종합해서 B형 간염과 D형 간염의 항원 항체 검사로 진단한다.
- D형 간염바이러스 항체(Anti-HDV antibody) 검사, 방사선 면역 측정법 검사, 효소 면역 측정법 검사 등으로 진단할 수 있다.
- 그 외 다른 간염과 감별 진단한다.
D형 간염의 치료와 예방
- 특효약이 없고 대증 치료를 한다.
- Pegylated Interferon- Alfa로 치료한다. Myrcludex으로 치료도 한다.
