- 그 소아의 어머니의 임신과 출산의 병력,
- 그 소아의 과거와 현재 병력,
- 가족 병력,
- 부모와 친 형제자매들의 가족 병력,
- 소아 예방접종에 관한 정보,
- 성장발육 이정표,
- 음식물 먹는 습관,
- 부모의 사회생활에 관련 사항 등의 정보를 기록할 수 있게 만든 소아 외래 건강검진 진료 기록지(I)가 있고
