터너 증후군의 원인

  • 태어날 때부터 성염색체 이상으로 생기는 증후군을 터너 증후군(Turner’s syndrome/Turner syndrome ) 또는 성선 발육부전증(性腺發育不全症)이라고 한다. 
  • 터너 증후군은 2500의 여아들 중 1명에게 발생한다. 
  • 정상 여아의 총 염색체 수는 46개이다. 46개 염색체 중 44개는 상염색체, 나머지 2개는 성염색체이다. 
  • 터너 증후군이 아닌 건강한 여아들의 핵형은 44XX이다. 
  • 터너 증후군 여아들의 41%의 총 염색체 수는 45개이고 그들의 핵형은 45XO이다. 나머지 59%의 핵형은 다른 종류이다(출처; Pediarics, December 1991 p.11480). 
  • 모자이크 염색체로 생긴 터너 증후군의 총 염색체 수는 45개와 46개이다. 이경우의 핵형은 45X/46XX이다. 
  • 45XO 핵형 터너 증후군 중 40%는 Y 염색체 성분이 조금 들어있다
  • 다시 말하면, 터너 증후군을 가진 여아들의 총 염색체 핵형이 45XO이거나 45X/46XX 모자이크형이다. p.00 염색체, p.00 성장 중지증의 내적 요인 참조. 
  • 터너 증후군은 부모의 성세포 감수분열 이상으로 생긴다. 

 터너 증후군의 증상 징후

  • 저 신장(100%), 난소기능 부전(90% ), 통통한 손 발(80%), 넓은 앞가슴(80%), 저 후두부 모발 선( 80%), 외이 기형(80% ) 적은 하악( 70% ), 몽고인 추벽(70% ), 팔꿈치 과신장(70%), 연 손톱(70%), 신장 기형(60%), 제 4지 단증(50%) 익상 목(50%), 색소모반(50%), 선천성 심장질환(50%), 난청(50%), 협소 고 아취 구개(40%) 등의 증상 징후가 생길 수 있다. 
  • 터너 증후군을 가진 갓 태어난 신생아의 체중은 정상 신생아들의 체중에 비해 좀 더 가볍다. 
  • 터너 증후군을 가진 갓 태어난 아기들의 손등과 발등이 부어 통통하다. 
  • 태어나서부터 영유아기, 학령기와 사춘기를 거쳐 성인으로 자랄 때까지 터너 증후군 여아들의 신장과 체중은 동년배들에 비해 더 작고 몸집도 전체적으로 더 작다. 
  • 손가락도 키도 짤막하고 체구는 오동 통통하다. 
  • 팔꿈치를 곧게 폈을 때 정상보다 더 많이 뒤로 젖혀질 수 있는 외반주 증상이 생긴다. 
  • 뒤통수 머리털이 뒷목 맨 아래 부위까지 날 수 있다. 이런 증상을 낮은 후 두부 두발선이라고 한다. 
  • 어깨와 목 사이가 날개 모양으로 생길 수 있다. 이렇게 생긴 목을 익상 목이라고 부른다. 
  • 양쪽 난소는 흔적만 있고, 
  • 대동맥 축착 등 선천성 심장혈관 기형이나 
  • 방패형 가슴, 
  • 사지 림프수종, 
  • 모반, 
  • 안근결손, 
  • 신장 기형 등이 생길 수 있다. 
  • 당뇨병이나 하시모토 갑상선염 등의 발생률이 높다.
  • 지능지수는 정상일 수 있고 정상 이하일 수 있다. 
  • 사춘기가 될 때 2차 성징이 동년배들보다 현저히 늦게 나타난다. 
  • 음모가 조금 나든지 거의 나지 않고 월경을 하지 않는다. 
  • 터너 증후군을 가진 여아들의 사춘기가 늦게 오고, 키가 작으며 그 외 다른 문제로 사회생활, 성관계를 하는 데, 정신 정서적면에서 문제가 생기기 쉽다. 
  • 터너 증후군이 있는 여아들의 청력 문제가 잘 생길 수 있다. 청력 이상이 생기지 않게 예방해 주지 않거나 청력문제를 적절히 해결해 주지 않으면 그들의 삶에 큰 지장이 생길 수 있다. 청력장애로 그들의 삶에 부정적으로 영향받는다. 

터너 증후군의 진단

  • 병력·증상 징후·진찰소견 등을 종합해서 터너 증후군이 의심되면 입 안 점막층에서 피검물을 채취해서 타원형 소체검사 (Barr body test)를 하고 염색체검사를 해서 진단한다. 
  • 혈중 FSH 농도, 항 마이크로소말 갑상선 항체, T4, 갑상선 자극 호르몬(TSH), 혈당, DNA검사 등으로 진단한다. 
  • 난소 초음파 검사를 하고 심장과 신장 등에 어떤 기형이 있나 알아보기 위해 보다 세밀한 진찰을 받아야 한다. 

터너 증후군의 치료

  • 난소가 없기 때문에 여성 호르몬이 분비되지 않는다. 
  • 13∼15세 경 부터 에스트로겐 여성 호르몬으로 성장 발육을 촉진시키고 월경을 하도록 도와준다. 
  • 월경을 시작한 이후부터 에스트로겐과 프로게스테론 여성 호르몬으로 일생동안 계속 치료받고 정상적인 성인 여성과 같이 주기적으로 월경을 하도록 도와준다. 
  • 선천성 신장 기형, 선천성 심장혈관 기형, 그 밖에 다른 여러 종류의 선천성 기형이 신체 다른 부위에 생길 수 있다. 세밀한 진찰을 받아 그런 선천성 기형이 있나 알아보고, 있으면 적절히 치료받는다. 
  • 4세경부터 성장호르몬 주사 치료를 받아 키가 크도록 한다. 
  • 주기적으로 월경을 할 수 있게 할 수 있지만 임신은 할 수 없다. 
  • 정신과 전문의의 치료가 때로는 필요하다.

Turner syndrome

Causes of Turner Syndrome

• Syndrome caused by a sex chromosome abnormality from birth is called Turner’s syndrome/Turner syndrome or gonadoplasia.

• Turner syndrome affects 1 in 2500 girls.

• The total number of chromosomes in a normal girl is 46. Of the 46 chromosomes, 44 are autosomal and the remaining 2 are sex chromosomes.

• The karyotype for healthy girls without Turner syndrome is 44XX.

• 41% of girls with Turner syndrome have a total chromosome number of 45 and their karyotype is 45XO. The remaining 59% of the karyotypes are different (source; Pediarics, December 1991 p.11480).

• The total number of chromosomes in Turner syndrome caused by mosaic chromosomes is 45 and 46. The karyotype in this case is 45X/46XX.

• 40% of 45XO karyotype Turner syndromes contain a small amount of the Y chromosome. • In other words, the total chromosome karyotype of girls with Turner syndrome is 45XO or 45X/46XX mosaic. See Chromosomes,  Intrinsic Factors in apoptosis.

• Turner’s syndrome is caused by an abnormality in parental sex cell meiosis.

Symptoms, signs of Turner Syndrome

• Low stature (100%), ovarian insufficiency (90%), chubby hands and feet (80%), broad pronotum (80%), low occipital hairline (80%), external ear malformation (80%), low mandible ( 70 % ), Mongolian vertebrae (70%), elbow hyperextension (70%), soft nails (70%), renal anomaly (60%), tetraplasty (50%) pterygoid neck (50%), nevus pigmentosa ( 50%), congenital heart disease (50%), deafness (50%), and narrow and high arched palate (40%) may develop.

• Newborns with Turner syndrome weigh slightly less than normal newborns.

• Newborn babies with Turner’s syndrome have swollen, plump backs of hands and feet.

• From birth to infancy, school age and puberty to adulthood, girls with Turner syndrome are smaller in height and weight and smaller overall than their peers.

• His fingers and stature are short, and his body is plump.

• When you straighten your elbow, you develop valgus, which can lead you back more than normal.

• The hair on the back of the head can fly all the way to the bottom of the back of the neck. This symptom is called low occipital baldness.

• The space between the shoulder and the neck may be wing-shaped. A neck like this is called a pterygoid neck.

• Both ovaries have only traces;

• Congenital cardiovascular anomalies such as aortic constriction

• shield chest;

• limb lymphedema;

• birthmarks;

• eye muscle defects;

• Kidney malformations may occur.

• The incidence of diabetes and Hashimoto’s thyroiditis is high.

• IQ may be normal or below normal.

• At puberty, secondary sexual characteristics appear significantly later than in peers.

• Little or little pubic hair and no menstruation.

• Girls with Turner’s syndrome come late to puberty, are short, and have other problems, such as social life and sexual intercourse, and are prone to psycho-emotional problems.

• Girls with Turner syndrome are more prone to hearing problems. If hearing problems are not prevented or properly addressed, their lives can be severely disrupted. Hearing impairment negatively affects their lives.

Diagnosis of Turner Syndrome

• If Turner’s syndrome is suspected by combining the medical history, symptom signs, and examination findings, a specimen is collected from the mucous membrane of the mouth, and an oval body test (Barr body test) is performed, and a chromosome test is performed to diagnose.

• Diagnosis is made by blood FSH concentration, anti-microsomal thyroid antibody, T4, thyroid stimulating hormone (TSH), blood sugar, and DNA test.

• A more detailed examination is needed to do an ultrasound of the ovaries and look for any abnormalities in the heart and kidneys.

Treatment of Turner Syndrome

• Because there are no ovaries, female hormones are not secreted.

• From around 13 to 15 years old, estrogen is a female hormone that promotes growth and development and helps menstruate menstruation.

• After the onset of menstruation, estrogen and progesterone continue to be treated for life, and help menstruate regularly like a normal adult woman.

• Congenital kidney anomalies, congenital cardiovascular anomalies, and many other congenital anomalies can occur in different parts of the body. Get a detailed examination to find out if there is such a congenital anomaly and receive appropriate treatment.

• From around the age of 4, receive growth hormone injections to grow taller.

• May be able to periodically menstruate, but not become pregnant.

• Seeing a psychiatrist is sometimes necessary.
