초경이 늦을 때(초경 지연), Delayed menarche

- 초경을 하는 사춘기 여아의 연령은 사춘기 여아의 체질, 선천성, 인종, 문명, 건강, 영양, 정신, 기후, 사는 나라, 사회 등에 따라 다르다.
- 대부분의 사춘기들은 9∼17세 사이에 초경을 한다.
- 초경을 하는 평균 연령은 12.5세이다.
- 일반적으로 16세가 될 때까지 초경을 하지 않으면 의학적으로 초경이 늦다고 한다. 사춘기가 늦은 아이들 참조.
- 사춘기 여아들의 일부는 10~11세에 초경을 하고, 사춘기 여아들의 62%가 10~13세 사이에 초경을 한다.
- 대부분의 사춘기 여아들은 적어도 16세가 되기 전까지 초경을 한다.
- 사춘기 여아들의 2%는 16~17세에 초경을 한다.
- 적어도 16세가 될 때까지 초경을 하지 않으면 초경이 늦은 원인을 알아보아야 한다.
초경이 늦는 원인
다음과 같은 원인으로 초경이 늦어질 수 있다.
- 선천성으로 자궁 기형이나 처녀막 기형이 있을 때
- 뇌·뇌하수체·난소·갑상선·부신 등 내분비선에 이상이 있을 때
- 정신 이상
- 스트레스를 너무 많이 받을 때
- 임신됐을 때
- 터너 증후군이나 그 외 염색체 이상
- 그 외
초경이 늦을 때 증상 징후
- 사춘기 여아가 만 16세가 될 때까지 아무 병이 없이 선천성으로 초경을 하지 않을 때는 초경이 늦는 것 이외 다른 증상 징후는 없는 것이 보통이다.
- 그렇지만 대부분의 동년 또래들은 이미 초경을 시작했고 그 후 월경을 하는데 자기는 초경도 하지 않았으면 많이 고민하게 된다.
- 어떤 병으로 초경이 늦거나 초경을 전혀 하지 않을 때는 그 병으로 인한 증상 징후도 함께 나타날 수 있다.
초경이 늦을 때 진단
- 병력·증상 징후·진찰소견 등을 종합해서 진단한다.
- 초경이 늦으면 호르몬 검사를 해서 선천성으로 초경이 늦는지,
- 어떤 병으로 늦는지 감별 진단해야 한다.
- 초경이 선천적으로 늦게 온 대부분의 사춘기 여아들의 어머니들의 대부분도 초경을 늦게 했었다는 가족 병력이 있고,
- 또 친언니들이나 여동생들도 초경을 늦게 했다는 가족병력이 있는 것이 보통이다.
초경이 늦을 때 치료
- 초경을 늦게 하는 원인이 선천적이 아니고 다른 원인으로 초경을 늦게 할 때는 그 원인에 따라 치료한다.
- 때로는 염색체검사를 한다.
- 초경이 선천적으로 늦게 올 때는 점차로 성장하면서 때가 오면 초경을 정상적으로 하는 것이 보통이다.
