소아 성 학대 (아동 성 학대/어린이 성폭력)

  • 여기서 말하는 소아와 소아청소년(0~18세)은 같은 의미를 지닌다.
    •  소아를 성적으로 희롱하거나, 
    •  소아와 성교를 하거나 
    •  소아를 강간해서 일방적으로 성적자극을 받게 하는 행위를 소아 성 학대라고 할 수 있다. 
  • 성인이나 18세 이하의 사춘기 아이가 저보다 나이가 어린 소아의 신체를 고의적으로 자기의 신체에 접촉해 소아와 상호작용해 소아로부터 성적 자극을 받는 행위를 소아 성 학대라고 미 질병관리 센터는 정의한다. 
  • 성인이나 다른 사람이 
    •  성적 자극을 받기 위해 소아의 신체를 추잡하게 접촉하거나, 
    •  소아의 신체를 강습하거나 
    •  음탕한 말이나 행위를 하거나, 
    •  성교를 하거나 강간을 하거나 
    •  항문 섹스를 하거나, 
    •  구강 섹스를 하거나 
    •  근친상간을 하는 행위도 소아 성 학대에 해당된다. 
  • 소아 성 학대의 대부분은 성인에 의해 이루어지지만 일부는 소아 성 학대를 당한 소아의 나이보다 더 많이 먹은 18세 이하의 사춘기 아이에 의해서도 이루어진다. 
  • 미국에서는, 소아가 강간을 당하거나 소아 신체를 강습하는 소아 성 학대를 당했다고 의심하거나 확인되면 미 각주 법에 따라 경찰 당국에 의무적으로 즉시 보고해야 한다. 
  • 소아가 단순 성희롱 성 학대를 당했다고 의심하면 소아 보호 관계당국에 보고해야한다. 그러나 사는 곳의 법에 따라 보고 하는 방법이 다르다.
  • 소아의 안녕과 건강 증진이나 교육에 관련된 직장에서 일을 하는 사람들이나 소아를 보호하고 보살피는 직장에서 종사하는 사람들 즉, 어린이집, 어린이 데이케어, 유치원 등에서 일하는 사람들, 교사들, 의사들, 간호사들은 소아가 성 학대를 당했다고 의심하면 소아 보호국에 보고할 의무가 있다. 
  • 소아가 낯모르는 사람에 의해 신체 접촉을 당해서 소아 성 학대를 받았을 때는 경찰당국에 보고해야 하는 것이 일반적이다. 
  • 각 나라나 각 지역 법에 따라 소아 성학대 관련법이 다르고 소아가 성 학대를 당한 사실을 당국에 보고해야 하는 의무와 방법도 다르다. 
  • 이와 같이 소아 성 학대는 사회적, 법적, 의학적 문제가 되고 그의 원인과 해결, 치료, 처치, 예방 방법이 복잡하다. 
  • 어린이 성 폭력은 소아 성 학대의 일종이었다. 
  • 미국에서 소아 성 학대를 당한 남여 소아 자신뿐만 아니라 학대를 한 사람, 소아 성 학대를 당한 소아의 가족에게도 정신적 정서적 문제가 생긴다. 
  • 소아 성 학대는 나라와 사회의 문제이고 나아가서는 현재뿐만 아니라 장래에도 소아 성 학대를 당한 본인에게 비극을 가져올 수 있다. 
  • 그 뿐만 아니라 소아 성 학대를 당한  소아의 정신 정서 육체적 문제로 이어질 수 있다. 
  • 소아학대를 받았던 경험이 있는 소아가 자라서 성인이 된 후 결혼생활과 부부 생활뿐만 아니라 일생 생활에까지 소아 성 학대로 입었던 정신적 상처로 인해서 인생 장애자가 될 수도 있다.
  • 소아 성 학대는 결국 가정 국가 사회의 문제로 이어진다. 

소아 성 학대의 종류 

소아 성 학대는 소아 성희롱 성 학대, 소아 성교 성 학대, 소아 강간 성 학대로 3분 할 수 있다. 

  ① 소아 성희롱 성 학대 (소아를 성적으로 희롱해서 성적 자극을 받는 행위

  • 가해자가 소아의 외음부를 만지거나, 
  • 가해자의 외음부를 소아에게 보이거나 만지게 하거나, 
  • 소아의 외음부를 보거나, 
  • 성교 행위를 하는 짓을 보이거나 
  • 성교 행위를 하는 짓을 하게 하거나, 
  • 성적 자극을 시킬 수 있고 성적 수치감을 야기시킬 수 있는 사진 등 음란물을 가지고 같이 보거나, 놀거나, 보이거나
  • 소아의 신체를 접촉해서 성적 자극을 받아 쾌감을 갖는 행위를 소아 성희롱 성 학대라고 한다. 

② 소아 성교 성 학대 (소아와 성교를 해서 성적 자극을 받는 행위) 

소아의 입, 질 또는 항문에 가해자의 외음부를 강제성을 띠지 않고 접촉시켜 성적 자극을 받는 행위를 소아 성교 성 학대라고 한다. 

③ 소아 강간 성 학대 (강제로 성교해서 성적 자극을 받는 행위) 

소아의 입, 질에 또는 항문에 가해자의 외음부를 강제로 접촉시켜 성적자극 받는 행위를 강간이라 한다.

소아 성 학대의 역학

  • 소아 성 학대를 
    • 가족 중 한 사람으로부터 받는 경우가 가장 많고, 
    • 그 다음으로 소아 성 학대를 받은 소아가 평소 잘 알고 지내는 이웃 사람이나 가족,
    • 가족의 친구나 친지로부터 더 많이 받고, 
    • 마지막으로 낯모르는 사람에 의해 소아 성 학대를 받기도 한다. 그 중 낯모르는 사람에 의해 소아 성 학대를 받는 경우는 아주 드물다. 
  • 2004. 10. 07 한국 성폭력 피해자에 관한 기사”에 의하면 한국 성폭력 피해자의 78%가 “아버지·삼촌·친오빠” 등 친족으로 부터 성학대를 당했다고 한다. 이 기사의 통계는 한국성폭력상담소 열림터에서 얻은 자료이다. 
  • 미국 소아청소년과 학회는 생후 4개월 이후부터 자녀는 엄마아빠의 침실에서 함께 자는 습관을 버리고 각 자녀들은 각 자녀의 침실에서 자라고 권장한다. 
  • 여아들의 20%, 남아들의 9%가 소아 성 학대를 받았다고 추정되고 
  • 소아 강간 성 학대는 전 소아 성 학대의 10%이하이고 
  • 소아들의 0.2~0.3%는 근친상간 소아 성 학대를 받고 근친상간 소아 성 학대가 5년 동안 계속 되기도 한다. 
  • 근친상간 소아 성 학대를 받은 소아들의 90%는 여아들이고 10%는 남아들이다. 최근 연구에 의하면 근친상간 소아 성 학대를 받는 소아들의 성별 차이가 점점 줄어들고 있다. 
  • 근친상간 소아 성 학대를 받은 소아들의 ⅓은 6세 이하, ⅓은 6~12세이고, ⅓은 12~18세이다. 
  • 한 아버지가 한 친 딸과 근친상간 성 학대를 한 다음, 다른 친 딸과 근친상간 성 학대를 하는 경우도 있다. 
  • 소아와 성교해서 소아 성 학대를 가한 사람들의 99%가 남성이고, 아이를 돌보는 사람으로부터 소아 성교 성 학대를 받을 때는 소아 성 학대를 한 사람이 여성인 경우가 많다. 
  • 친아버지에 의해 소아 성교 성 학대를 받는 수 보다 의붓아버지에 의해 소아 성교 성 학대를 받는 수가 5배나 된다. 
  • 1998년에 103,845명의 미 소아들이 소아 성 학대를 받았고, 
  • 1999년에 93,338명의 소아들이 소아 성 학대를 받았다. 그중 여아들 1,000명 중 1.6명, 남아들 0.4명꼴로 받은 셈이다. 
  • 2004년에 84,000명 이상 미 소아들이 소아 성 학대를 받았다 한다(출처-Pediatric News, July 2007). 

 소아 성 학대의 원인

  • 근친상간은 친 아버지와 친 딸 사이에 가장 많이 생겼고 한 번 근친상간이 이루어진 후에는 성폭행이 없이 상후 합의하에서 점차적으로 계속 이루어지는 것이 보통이다.
  • 때로는 친 아버지도 친 딸도 다 같이 성적욕구에 따라 근친상간이 계속 이루어질 수 있다.
  • 가정에서 성 폭력적 행위를 자행하는 아버지의 경우,
    • 부인이 장기간 정신적으로나 육체적으로 앓거나, 
    • 옥외 일을 많이 하거나, 
    • 우울증에 장기간 빠지거나, 
    • 아버지가 정서적으로 미숙하거나
    • 소아기 때 소아 성 학대를 받은 경우 친 딸과 근친상간을 하기가 더 쉽고 소아 성 학대도 할 수 있다. 
    • 근친상간을 정신병을 앓는 아버지가 더 하기 쉽다.
    • 경제적으로 빈곤하거나 엄마가 교육을 많이 받지 않았거나 
    • 아빠가 알코올 중독에 걸려있거나 약물 중독에 걸려있거나, 
    • 정신병이 있을 때 근친상간을 하기가 더 쉽다. 

소아 성교 성 학대의 증상 징후

  • 아버지나 가족의 일원에 의해 소아 성교 성 학대를 받았던 여아는 자기 엄마에게 성 학대를 받았던 사실을 직접 실토할 때가 많다. 엄마가 딸을 데리고 병원에 가서 의사의 도움을 청하는 경우도 있다.
  • 딸이 친 아빠로부터 성교 성 학대를 받았다는 사실을 엄마한테 보고해도 엄마가 그 사실을 받아드리지 않으면 성학대를 받은 어떤 딸은 자기의 여자 친구의 엄마나 학교 카운슬러 등에게 그 사실을 실토하는 경우도 있다. 
  • 소아 성교 성 학대를 받은 사춘기 여아는 자신의 의사에게 그 사실을 실토하고 의사로부터 도움을 구하는 경우도 있다. 
  • 소아 성교 성 학대나 강간 성 학대를 당하지 않고 입에 외음부를 접촉하는 정도로 성 학대를 받거나 추잡한 신체적 접촉 성희롱 성 학대를 받았을 때는 성 학대를 받았던 소아의 신체에 확연한 부상이 나타나지 않는 것이 보통이다. 
  • 때로는 가해자가 소아에게 뇌물을 주어 소아로부터 호감을 사거나 소아 성교 성 학대를 받아서 생긴 어떤 증상 징후를 아무에게도 실토하지 않도록 위협 하는 경우도 있다. 
  • 외음부 열창, 외음부 혈종, 외음부 부종, 처녀막 열상, 질 강 열창, 질 강 상흔, 이빨로 물린 자국, 발적, 찰과상, 자반 등이 소아 성교 성 학대로 인해 소아 성 학대를 받은 여아의 외부부 등에 나타날 수 있다.
  • 피부나 입 안 점막에서 발적, 찰과상, 자반 등이 발견될 수 있다. 
  • 그 외 항문 외상, 성병, 질 출혈, 질 분비, 재발성 요도염, 요로 감염, 야뇨증, 요분증, 매춘, 가출, 약물 중독, 자살기도, 수면장애, 복통, 학습 장애, 학교 성적 저하, 우울증, 변비증, 설사 등의 증상 징후가 생길 수 있다. 
  • 사춘기 여아는 성교 성 학대를 받은 후 임신 될 수 있다. 
  • 소아 성교 성 학대를 받은 
    • 3~8세 된 유아들이나 학령기 아이들의 28~38%는 악몽을 꾸고, 
    • 5~11세 된 유아들이나 학령기 아이들의 26%는 어둠 공포증을 갖고, 
    • 14%는 침실 공포증을 갖고, 
    • 4세  유아들의 30%와 6세 유아들의 10%에게 야뇨증이 생길 수 있고, 
    • 신체적 증상 징후와 행동 장애 등이 생길 수 있다
  • 사춘기가 되기 이전 소아청소년들에 생긴 성병은 거의가 소아 성교 성 학대로 인해 생긴다. 
  • 소아 성교 성 학대를 가한 사람이 그 사실을 비밀로 지켜달라고 강요했기 때문에 소아 성교 성 학대를 받은 사실이 뒤늦게 알게 될 때도 많다. 
  • 소아 성교 성 학대를 받은 소아청소년들의 신체검사의 결과는 
    • 15~85%에서 정상이고, 
    • 14~34%에서 외음부가 발적되어 있고, 
    • 15~85%에서 처녀막의 질구가 비정상적으로 크게 확장되고, 
    • 6~16%에서 외음부에 부상이 발견되고, 
    • 3~6%에서 성병이 진단되고, 
    • 2~18%에서 정자나 산성 인산효소 검사의 결과가 양성으로 나타난다.

다음과 같은 증상 징후나 행동장애가 소아에게 있으면 소아 성교 성학대를 받았는지 의심해 봐야 한다.

  • 엄마아빠로부터 떨어지지 않고 매달릴 때
  • 분노 발작
  • 공격적 행동,
  • 수면장애,
  • 악몽,
  • 식욕상실,
  • 너무 많이 먹거나 아주 안 먹거나,
  • 신경질환,
  • 공포증,
  • 행동장애,
  • 위축,
  • 우울증,
  • 자기비하,
  • 자해,
  • 또래들 간 사교문제,
  • 습관성 약물남용,
  • 학교문제,
  • 난잡한 성행위,
  • 매매춘,
  • 성학대 등 증상 징후가 있을 때

Child sexual abuse

• The terms children and adolescents (0-18 years old) have the same meaning here.

Types of Child Sexual Abuse

Child sexual abuse can be divided into three categories: child sexual harassment and sexual abuse, child sexual abuse, and child rape and sexual abuse.

① Child Sexual Harassment Sexual abuse (act of sexually harassing a child and receiving sexual stimulation)

• The abuser touches the child’s vulva;

• Have children show or touch the perpetrator’s vulva;

• Seeing the child’s vulva;

• Showing sexual intercourse or

• make you engage in sexual acts;

• Viewing, playing with, viewing, or sharing pornography, including pictures, that may be sexually arousing or sexually humiliating.

• The act of touching a child’s body and receiving sexual stimulation for pleasure is called child sexual harassment and abuse.

② Child sexual abuse (act of sexual stimulation by having sexual intercourse with a child)

The act of sexual stimulation by touching a child’s mouth, vagina, or anus with the perpetrator’s vulva without coercion is called child sexual abuse.

③ Child rape and sexual abuse (acts of sexual stimulation through forced intercourse)

Rape is the act of forcibly touching a child’s vulva to the child’s mouth, vagina, or anus to receive sexual stimulation.

Epidemiology of Child Sexual Abuse

• Child sexual abuse

o Most often from one member of the family,

o The next child who has been sexually abused is a neighbor or family member they know well;

o Get more from friends and relatives in your family, o Lastly, you may be sexually abused by someone you don’t know. Among them, cases of child sexual abuse by strangers are very rare.

• 90% of children subjected to incest child sexual abuse are girls and 10% are boys. Recent studies have shown that the gender gap among children who are subjected to incestuous child sexual abuse is shrinking.

• One-third of children who are sexually abused by incest are under the age of 6, one-third are between the ages of 6 and 12, and one-third are between the ages of 12 and 18.

• In some cases, a father sexually abuses one biological daughter, then incestuously abuses another biological daughter.

• 99% of those who perpetrate child sexual abuse by having sexual intercourse with children are men, and when child sexual abuse is received by caregivers, the perpetrators of child sexual abuse are often women.

• Child sexual abuse by stepfathers is five times more common than by biological fathers.

Causes of Child Sexual Abuse

Symptoms, signs  of Childhood Sexual Abuse

• A girl who has been sexually abused by her father or a member of her family often confesses to her mother that she was sexually abused. Sometimes her mother takes her daughter to the hospital and asks for help from the doctor.

• If she reports to her mother that her daughter was sexually abused by her biological father and the mother does not accept the fact, any daughter who has been sexually abused will tell her girlfriend’s mother, school counselor, etc. In some cases, confession.

• Adolescent girls who have been sexually abused by children sometimes confess to their doctor and seek help from her doctor.

• Child Sexual Abuse or Rape Sexual Abuse, Sexual Abuse, Sexual Abuse to the extent of mouth-to-mouth or indecent physical contact.

• Child sexually abused o 28-38% of children aged 3-8 years or school-age children have nightmares,

o 26% of infants and school-age children aged 5-11 years have a phobia of darkness;

o 14% have bedroom phobia, o Enuresis can occur in 30% of 4-year-old infants and 10% of 6-year-old infants;

o Physical symptoms, signs and behavioral disturbances may occur.

• Most sexually transmitted diseases in children before puberty are caused by sexual abuse of children.

• Child sexual abuse is often discovered late because the perpetrator of child sexual abuse forced it to be kept secret.

• The results of the physical examination of children and adolescents who have been sexually abused o Normal in 15-85%,

o In 14-34% of cases, the vulva is red, o In 15-85% of cases, the vaginal opening of the hymen is abnormally large,

If a child has any of the following symptoms or behavioral disorders, it should be suspected that the child has been sexually abused:

• When hanging from mom and dad without falling

• sleep disorders;

• nightmare,

• loss of appetite;

• Eating too much or not eating too much;

• neurological disorders;

• phobias;

• behavioral disorders;

• atrophy;

• depression,

• self-deprecation;

• self-harm;

• social issues among peers;

• habitual substance abuse;

• school problems;

• promiscuous sexual activity;

• prostitution;
